The DD Form 3145-4 is a required self-certification checklist for military spouses applying to DoD positions under the Priority Placement Program (PPP) military spouse preference.

Academic records showing courses, dates, degree, and GPA. Required when education is used to qualify for a position or substitute for experience.

Your most recent official performance rating from your federal supervisor. Required for current and former federal employees applying under merit promotion.
